今年要带几个阿姨出去考OW(我不是教练是悲催阿姨团长),其中有一位是中老年(大于50岁)+高血压。高血压的情况是:每天早上吃一次药控制血压晨峰(moming surge),然后一天血压无问题。
PADI的健康声明书上有高血压这块内容,但没找到完全不能潜的说法。我知无论如何要遵医嘱且看阿姨自己的意愿,建议这位阿姨去海军总医院看“航空与潜水治疗中心”了,同时要她把降压药名字给我,我问下DAN。海总这个治疗中心不知道靠不靠谱,不确定DAN会鸟我。以下资料供参考。

HYPERTENSION
Description of Condition: Hypertension, or high blood pressure, is one of the most common medical
conditions seen in the diving population - no surprise,really, since it is a common medical condition in the general population. Strict criteria for hypertension can vary depending on the reference cited, but normal blood pressure is generally accepted to be a systolic pressure below 140 and a diastolic pressure below 90 mm Hg, depending on age (cited as systolic first and diastolic second. eg. "120 over 80," by your doctor). A thorough medical evaluation should be performed to find a treatable cause for hypertension; in most cases, however, none will be found.
Basically, two different sets of complications face a person with hypertension: short-term and long-term. Short-term complications are generally due to extremely high blood pressure; the most significant is the risk of a stroke due to rupture of blood vessels in the brain (called a cerebrovascular accident). Long-term detrimental effects are more common: they include coronary artery disease, kidney disease, congestive heart failure, eye problems and cerebrovascular disease.
Fitness and Diving Issue: As long as the individual's blood pressure is under control, the main concerns should be the side effects of medication(s) and evidence of end-organ damage. Divers who have demonstrated adequate control of blood pressure with no significant decrease in performance in the water due to the side effects of drugs, should be able to dive safely. A recent report in a diving medical journal citing several episodes of acute pulmonary oedema (ie. lungs congested with fluid) in individuals with uncontrolled hypertension while they were diving. Regular physical examinations and appropriate screening for the long-term consequences of hypertension such as coronary artery disease are necessary.
Medication used in Treatment: Mild hypertension may be controlled with diet and exercise; however, medication is often necessary. Many classes of drugs are used to treat hypertension, with varying side effects. Some individuals must change medications after one drug appears to be or becomes ineffective. Others might require more than one drug taken at the same time to keep the blood pressure under control. Classes of drugs known as beta-blockers often cause a decrease in maximum exercise tolerance and may also have some effect on the airways. This normally poses no problem for the average diver. ACE (angiotensin converting enzyme) inhibitors are the preferred class of drugs for treating hypertensive divers; a persistent cough is a possible side effect. Calcium channel blockers are another choice, but lightheadedness when going from a sitting or supine position to standing may be a significant side effect. Diuretics are also frequently used to treat hypertension. This requires careful attention to hydration and electrolyte status. Most anti-hypertensive medications are compatible with diving as long as the side effects experienced by the diver are minimal and their performance in the water is not significantly compromised. Any diver with long-standing high blood pressure should be monitored for secondary effects on the heart and kidneys.

Can I dive with high blood pressure?
Yes, if your blood pressure is controlled with treatment which will not affect your fitness to dive. If your blood pressure on treatment is less than 160/90 and there is no clinical evidence of damage to the heart, kidneys or blood vessels then you can dive if your medication is listed as allowable:
If you are taking more than two allowable drugs (including combination preparations) for control of your blood pressure, or if you are taking medication in the second column (above) then you should be assessed by a medical referee before diving. Such assessment may include an exercise test to prove that your heart rate is not limited by the medication, and an ultrasound scan of the heart (echocardiography) to make sure that the heart muscle has not been affected by the blood pressure.

回来了。9月两位阿姨开心顺利地成为合格潜水员不说,还一举拿下AOW,还一直问薄荷怎么样啊我们也要去墨宝。。。
关于中老年人及高血压我观察到的:
高血压:未见影响。正常吃药正常潜水。
中老年:未见影响。
看到的问题:阿姨虽然已是AOW潜水员,但是下水还是相~当~的那个紧张。和平静水域展现出的放松自在完全相反,开水中他们身体僵硬,处理问题不熟练,极度依赖教练。当然这是所有新潜水员都会经历的阶段,不能完全归类到“年龄大接受新鲜事物较困难”这种高大上的观点中。
总的来说通过阿姨们的旅程,我觉得在药物可控制,未见药物相互影响的基础上(最好有医生证明),中老年+高血压可以正常参加潜水课程。可能学习进度会相对较慢,需要较长时间适应开放水域和放松身体。

唉这个阿姨还是高龄+高血压,更加的不确定了。
个人做法,找这位阿姨的家庭医生填写健康声明,看医生准不准这位阿姨潜水。PADI健康声明第二页(A-6)需要医生支持/反对及评论这位阿姨是否适合潜水。
我也看到PADI的健康声明书了,国内的话北京无专业潜水医生,另外都怕担责任估计都会选择保守观点。。。
让阿姨自己衡量去了,目前人还是要学,额。。。

最后结果如何?怎么木有汇报成果?
80岁可以浅吗
潛水20多年,還沒遇到這樣的問題,每年都要做身體檢驗,句括:
1)驗尿
2)驗聽覺
3)驗肺氣量
4)心電圖
5)量血壓
6)動態後心跳測試
如果經過5分鐘的跑樓梯,心跳脈搏測試高過180,就不能潛水了,
怎麼算:
分三次,每一分鐘計一次。
如果給你吃抗压药後,超過標準80/140,就一定不給潛水!
7)專科醫生檢查身體

没汇报是因为还木有学成,开放水域要在9月中旬完成。
目前北京下平静水域的情况:
两个阿姨因为经常游泳不怕水,平静水域1出奇的顺利,比我见过的青壮年第一次平静水域都好。学习认真,情绪高涨,真是发自内心的热爱下水。
没有看到两个中老年人和青年学潜水有什么不同,血压无异常。
8月下剩下的平静水域,9月开放水域。待两位阿姨拿证归来必详细汇报~
问了下周围的医生,不建议潜水,因为难保大妈下水一激动,就高了。

是的,年龄不是阻碍你探求神秘水底世界的障碍,老头我2017年底去宿务的side effects潜店考了ow,畅游了海底。
是的,学了可能没问题,但潜水前的健健康声明,怎么处理的?
如果没有医生声明而在声明中隐瞒高血压情况,如果出问题的话,所有责任都在个人了。。。
如果还打算持续潜水,可以考虑请dan推荐国内外可以开具证明的医生,方为上策。
